Case 7 ECG Posted April 5, 2020 by Jonni Cooper ​ This ECG was recorded on a 59 year old male patient with chest discomfort. What is your diagnosis? Submit your answer below in the space provided. ​ Answer to Case No 6 Posted November 2019 ...See ECG below...No correct answer was submitted for this one. The ECG was recorded from a patient with an unknown history at a routine office visit to his general practitioner. Interpretation: 1) Sinus rhythm (rate 90/min) with P-Pulmonale (P waves 3-4 mm tall in leads 2, 3, and aVF; P-axis +75 degrees) 2) Left axis deviation (axis -45 degrees), possible left anterior hemiblock with the rS pattern present in leads 2, 3, and aVF and a qR pattern in leads 1 and aVL. 3) Anteroseptal infarction of uncertain date, probably recent and evolving Comments: 1) Despite the ST elevation, without knowledge of the clinical history one cannot assume that the infarction is acute; for example, this could be an old anteroseptal infarction with a residual aneurysm. 2) The QRS axis is only about -45 degrees and there is not the increased voltage one would expect with LAHB; so that diagnosis is tentative. Fake Cardiology "Board" Warning The American Board of Cardiovascular Medicine (ABCM) has received dozens of phone calls and emails from Cardiologists who have received letters from a fraudulent group operating under the names of the American Board of Cardiovascular Diseases, and most recently, they have taken to using our namesake of the American Board of Cardiovascular Medicine, demanding that Cardiologists pay $500 in order to maintain their Cardiology License. Although we are the American Board of Cardiovascular Medicine, Inc., rest assured we are not associated with this group.
